Graham-Massey Toxicology Services and Procedures

Supplies:  All bottles, labels, security seals, requisition forms and mailers are supplied by Graham-Massey.  Supply
orders should be placed several days in advance of when they are needed.

Specimen Transportation:  Pick up of specimens by Graham-Massey is available on a daily basis within Connecticut
and Massachusetts.  Specimens from other states are sent via first class mail or overnight courier.

Turnaround Time:  All specimens are analyzed and reports issued within 24 hours of arrival at the laboratory,
excluding weekends and holidays.  Special request testing may require an additional 24-48 hours.

Test Reports:   Immediately upon completion of testing, positive results are reported via fax, email, internet, phone or
remote printer unless other arrangements are made.  Written reports of results are delivered as requested.

Repeat Testing:  Graham-Massey will provide repeat testing if required.  All specimens are retained for seven days
after the original test date, or in accordance with the clients’ requirements.  In order to assure the availability and
integrity of specimens, recheck requests should be made as soon as possible after the receipt of the original test
report.

Technical Consultation:  The director and technical staff at Graham-Massey are available to provide technical
assistance to our clients.  At Graham-Massey we believe that a laboratory has an obligation to provide its clients with
more than a simple positive or negative result.  Our entire staff stands ready to assist clients  in any way possible to
make our results meaningful and clinically significant.

In-Service Workshops:  As a specialty drug testing laboratory, Graham-Massey is uniquely qualified to provide
information on all aspects of the drug testing process.

Courtroom Testimony:  Graham-Massey will assist clients in responding to legal challenges related to results provided
by our laboratory.  Provided sufficient notice is given, we will furnish affidavits and/or make our staff available for
testimony at legal proceedings.

Chain of Custody:  To facilitate specimen chain of custody and provide a clear audit trail, Graham-Massey utilizes
uniquely numbered requisition forms.  Graham-Massey also provides an outer security seal which is placed over the
container cap following collection of the specimen.  These devices serve to demonstrate the integrity of the container
following collection.

Security:  All interior areas of the facility are monitored with motion and smoke detectors during non-working hours.  
While occupied, all areas with exception of the visitor reception are kept secured with combination pass locks on the
inner doors.  All visitors must check in at the reception desk and be accompanied by a staff member when passing
beyond the reception area.

When a specimen arrives at the lab, it is assigned a unique accession number.  The accession number is recorded on
the requisition form and the specimen container, and on all vessels and worksheets related to any test performed on
the specimen.  Specimens are sampled separately from the original container for each phase of testing.  Each sampling
involves a cross reference or the requisition and accession number on the specimen container and worksheet.

Specimen Storage:  Graham-Massey retains all urine specimens for seven days.  When required by state law or
requested by clients, a specimen will be maintained for an additional period of time.  Extended freezer storage is
available.

Confidentiality:  Graham-Massey acknowledges that in receiving, storing or otherwise dealing with any information
from its clients, it is bound by State and Federal laws regarding confidentiality.  An identifying number may be used in
place of the contributor’s name, thereby shielding the identity of the contributor from laboratory staff.

Quality Assurance/Quality Control:  Graham-Massey utilizes open quality materials to ensure adequate performance
at all stages of analysis, from specimen processing to final reporting.  Additional checks and balances have been
implemented to monitor turnaround times and accuracy of clerical entries.

External Quality Assurance:  Graham-Massey participates in the trimester drug screening proficiency survey
conducted by the College of American Pathologists (CAP).  CAP is an independent nonprofit association which provides
proficiency testing samples to laboratories throughout the country on a subscription basis.  Results of all participating
laboratories are tabulated by CAP and a repot sent to each lab.  This allows each participating laboratory to judge its
proficiency relative to others.  We take great pride in having maintained a 100% proficiency rating in surveys since 1983.

Special Requests:  Graham-Massey will make every effort to provide additional testing which may be required by our
clients but is not a part of our normal test menu.
